This Release and Waiver of Liability (the “Release”) executed by the
“Participant” in favor of RVA Hand Up of Chesterfield, a non-profit
organized and existing under the laws of the state of Virginia, USA, its directors,
officers, employees, volunteers, and agents (collectively, “RVA Hand Up of Chesterfield”).
I, the Participant, desire to serve as a volunteer and/or accompany other volunteers to further the work of RVA Hand Up of Chesterfield in its community outreach initiatives. I acknowledge that I am acting as a volunteer and undertake to perform said services without compensation and not as an employee of RVA Hand Up of Chesterfield.
I hereby freely and voluntarily, without duress, execute the Release under the following terms:
1. Waiver and Release. I, the Participant, release and forever discharge and hold harmless RVA Hand Up of Chesterfield from any bodily injury, personal injury, illness, death, or property damage that may result from my participation as a volunteer. I also understand that RVA Hand Up of Chesterfield does not assume any responsibility or obligation to provide financial or other assistance, including, but
not limited to medical, health, or disability insurance, in the event of injury, illness, death, or property damage.
2. Insurance. RVA Hand Up of Chesterfield does not carry or maintain, and expressly disclaims responsibility for providing any health, medical, or disability insurance coverage for the Participant. EACH PARTICIPANT IS EXPECTED AND ENCOURAGED TO CARRY PERSONAL LIABILITY OR HEALTH INSURANCE PRIOR TO REGISTERING AS A RVA HAND UP OF CHESTERFIELD VOLUNTEER.
3. Medical Treatment. Except as otherwise agreed to by RVA Hand Up of Chesterfield in writing, I hereby release and forever discharge RVA Hand up of Chesterfield from any claim whatsoever that arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time with RVA Hand Up of Chesterfield.
4. Assumption of Risk. I understand that my time with RVA Hand Up of Chesterfield may include activities that may be hazardous to me, including, but not limited to, cooking and food preparation activities, loading and unloading of heavy equipment and materials, and transportation to and from sites. I recognize and understand that my time with the RVA Hand Up of Chesterfield may in some situations, involve inherently dangerous activities. I hereby expressly and specifically assume the
risk of injury or harm in these activities and release RVA Hand Up of Chesterfield from all liability for injury, illness, death, or property damage resulting from the activities of my time with RVA Hand Up of Chesterfield. To express my understanding of this Release, I have signed the document below. *** If any school volunteers on behalf of the students, then the school assumes the responsibility and risk of any situations that may arise from the activities. RVA Hand Up of Chesterfield will not be held liable***
I grant permission to RVA HAND UP OF CHESTERFIELD for the use of the photograph(s) or electronic media images in any presentation of any and all kind whatsoever. I understand that I may revoke this authorization at any
time by notifying RVA HAND UP OF CHESTERFIELD in writing at rvahandupofchesterfield@yahoo.com. They will be kept as long as they are relevant and after that time deleted.
